Here are the steps to fill out the renewal:

  1. Access the external website: https://kai02.qwest.com/pswa/badgestart.aspx
  2. Answer the Question “Are you a Qwest Employee? “ No
  3. Click Continue
  4. Read the sentence that begins  ** If you do not have a password to login please click here……then click the link here
  5. Complete the form.  Be sure to provide your email and use john.dudek@qwest.com as your sponsor.
  6. Re- Access the external website: https://kai02.qwest.com/pswa/badgestart.aspx
  7. Answer the Question “Are you a Qwest Employee? “ No
  8. Enter email used previously and new password received in appropriate fields.
  9. Change password and write it down for future reference
  10. Select I am applying for myself. from #1
  11. Select Badge from # 2a
  12. Select Renew an existing facility access badge in 2B
  13. click Continue
  14. enter Your Last Name and Badge #,  (Found on the back
  15. Click Continue
  16. Check that Sponsor is Cindy Barnes Pharr If yes then click Proceed.  If not Enter sponsor email (For all pioneer requests it is
                  cindy.BarnesPharr@qwest.com
          
  17. Click Continue
  18. Click Proceed
  19. Al previous info should appear  but fill in any blanks you can.
    1. address, ( use 310 SW Park avenue, Flr 4  for your address ) 
    2. telephone, ( Use the 242-7673 as the business phone)
    3. Fill out the rest as appropriate
  20. Click Continue
  21. Select Badge Applicant is a Non-Qwest Employee
  22. In company name type Qwest Pioneer
  23. Select General for Badge type
  24. Select All Days of the Week
  25. Select 24 hours
  26. In access Locations Select:
    If this Renewal application does not require change in current access, then check below.
    Check here to retain current access.
  27. In Reason for Access Select Other  and enter Pioneer volunteer in the Text Box
  28. click submit
